It really can happen just that fast and of course in close quarters.  You need to be aware and in the situation to have a chance.  Even then good luck as odds are not going to always be in your favor.  One crucial thing to remember is to watch people’s hands with your peripheral vision.  If you can’t see their hands you may have a problem!
